การส่งตรวจทางห้องปฏิบัติการ ถือเป็นสิ่งจำเป็นต่อการวินิจฉัยโรคและการติดตามผลการรักษา
อย่างไรก็ตาม ทุกการส่งตรวจมีต้นทุน การตรวจที่มากเกินความจำเป็นอาจส่งผลเสีย ไม่ว่าจะเป็นการเจ็บตัวจากการเจาะเลือด, ค่าใช้จ่ายที่เพิ่มของผู้ป่วย และยังทำให้ปริมาณงานส่งตรวจของโรงพยาบาลมากเกินจนส่งผลต่อความรวดเร็วของผลตรวจของผู้ป่วยที่จำเป็นจริงๆอีกด้วย แต่การส่งที่น้อยเกินไปก็อาจส่งผลให้การวินิจฉัยและการติดตามการรักษาไม่ดีเท่าที่ควร
การส่งตรวจที่เหมาะสม จึงเป็นการส่งตรวจที่มีความพอดี มีจุดประสงค์ที่ชัดเจน และมีผลต่อการวินิจฉัย วางแผนการรักษา หรือติดตามผู้ป่วย นั่นเอง
Laboratory testing is crucial for diagnosis and treatment monitoring. However, excessive testing can lead to unnecessary procedures, higher costs, and delays in results for those who need them. On the other hand, insufficient testing may compromise diagnosis and care.
The key is to order tests that are necessary, with clear objectives, and directly impact diagnosis, treatment, or patient monitoring.

"สมเหตุสมผล"
โรงพยาบาลจุฬาลงกรณ์สภากาชาดไทย ร่วมกับคณะแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัย ได้จัดตั้งโครงการ Rational Laboratory Use in KCMH เมื่อวันที่ 20 ม.ค. 2565 โดยมีวัตถุประสงค์เพื่อให้การส่งตรวจในโรงพยาบาลจุฬาฯ เป็นไปอย่างเหมาะสม เพื่อประโยชน์สูงสุดแก่ผู้ป่วย
King Chulalongkorn Memorial Hospital, in collaboration with the Faculty of Medicine at Chulalongkorn University, launched the Rational Laboratory Use in KCMH project on January 20, 2022. The project aims to ensure appropriate lab testing at the hospital, maximizing benefits for patients.

Each patient’s care is unique, it is impossible to establish rigid guidelines for testing that apply to all patients. Considering the necessity of tests and the benefits they will provide to each patient before ordering is the most critical aspect of the Rational Lab Use project.

เนื่องจากรูปแบบการดูแลผู้ป่วย รวมถึงการส่งตรวจทางห้องปฏิบัติการ อาจมีความแตกต่างกันในแต่ละสถานการณ์ โครงการของเราจึงแบ่งกลุ่มคณะทำงาน ออกเป็น 4 กลุ่ม เพื่อการออกแบบนโยบายที่ตอบโจทย์การดูแลผู้ป่วยแต่ละรูปแบบ ได้แก่
As patient care and laboratory testing may vary in different situations, our project divides the team into four groups to design policies tailored to each patient care model, including:
OPD
ลดการส่งตรวจทางห้องปฏิบัติการของผู้ป่วยนอกที่ถี่เกินความจำเป็น เช่น การตรวจน้ำตาลสะสม HbA1c ที่ต่ำกว่า 84 วัน และลดการตรวจที่ซ้ำซ้อน เช่นการตรวจไขมัน Direct LDL ใน Lipid profile
Reduce unnecessary frequent laboratory testing for outpatients, such as HbA1c tests within 84 days, and minimize redundant tests like Direct LDL in lipid profiles.
IPD
ลดการส่งตรวจทางห้องปฏิบัติการของผู้ป่วยใน ที่ซ้ำซ้อนและมากเกินความจำเป็น ลดการสั่งเป็นชุด เช่น ชุดการตรวจ Liver Function Test, การตรวจ Coagulation, การตรวจ Ca Mg PO4 หรือชุดการตรวจ Electrolytes
Reduce redundant and excessive laboratory testing for inpatients, minimizing the use of test panels such as LFT, coagulation test and bundled electrolytes testings.
Checkup
สนับสนุนการส่งตรวจอย่างเหมาะสมสำหรับผู้ป่วยที่ตรวจสุขภาพทั่วไป ตามอายุของผู้ป่วย โดยร่วมมือกับภาควิชาเวชศาสตร์ป้องกันและสังคม โดยสร้างชุดตรวจในระบบคอมพิวเตอร์สำหรับแพทย์
Support appropriate testing for general health checks based on patient age, in collaboration with the Department of Preventive and Social Medicine, by creating test panels in the physician’s computer system.
Pre-Op
สนับสนุนการส่งตรวจทางห้องปฏิบัติการสำหรับผู้ป่วยที่เตรียมตัวผ่าตัด ที่เหมาะสมกับอายุ โรคประจำตัว และชนิดของการผ่าตัด โดยร่วมมือกันระหว่างภาควิชาวิสัญญี และภาควิชาอื่นๆที่มีการผ่าตัด
Support suitable laboratory testing for patients preparing for surgery, tailored to their age, underlying conditions, and type of surgery, in collaboration with the Department of Anesthesiology and other surgical departments.

KCMH utilizes a fully computerized testing system. Our health information system connects both physician orders and diagnostic analyses, allowing for tailored approaches based on the patient's specific disease and circumstances.

ปัจจุบัน โครงการได้นำร่องแนวทางการส่งตรวจที่เหมาะสมไปใช้ในหน่วยต่างๆในโรงพยาบาลจุฬาลงกรณ์ ครอบคลุมการดูแลผู้ป่วยทั้ง 4 กลุ่ม เรามีระบบแจ้งเตือนสำหรับแพทย์ มีการประชุมคณะทำงานสม่ำเสมอ เพื่อระบุปัญหา การสั่งตรวจ และวางแผนนโยบายได้อย่างตรงจุด
The project is successfully implementing appropriate testing protocols across King Chulalongkorn Memorial Hospital, encompassing all four patient care groups. With an effective alert system for physicians and regular team meetings, we address challenges in test ordering and enhance patient care.
